pdf links keep breaking

I love the concepts behind Mendeley but I've been struggled for years with disappearing links between Mendeley Desktop and the PDF's on my computer. I recently came back to give Mendeley another shot but this appears to still be broken, and I'm at a loss as to what to do. I have many hundreds of broken connections between my metadata records and my PDF's. I decided to post about it because I experienced a striking case of this problem over the past week. (1) I added a document to Mendeley by dragging it in from my computer a few days ago (2) Today I opened Mendeley to find that no PDF was linked to that metadata record (3) I manually relinked the article (4) I clicked "Sync" (5) The article link disappeared (there was no longer a PDF associated with the metadata) (6) I tried dragging the article back into Mendeley as was suggested in another post, hoping it would automatically reconnect to the metadata record (7) A new, duplicate entry was created for the article (8) Even thought I've spent a lot of time managing my own folder of articles, I tried having Mendely copy all my PDF's and organize them it's own way, but the problem still occurs. I've got many hundreds of metadata records that have broken links to the PDF files (even though I've NEVER moved or modified the PDF files), and I'm at a loss for how to fix all these broken links and, even more important, how to avoid this problem in the future. All the in-document notes I've made on the PDFs with disappeared links appear to be lost, as far as I can tell. I'm using OSX 10.8.4 on Mendeley 1.9.1
